Actigram is a digital therapeutics platform for people with or at risk for cardiovascular disease (CVD) that helps create long term behavior change through habit formation and better day-to-day lifestyle decisions.
The platform enables physicians and patients to collaboratively assess CVD risk and set goals such as walking, improving diet, avoiding saturated fats, and adhering to medication regimens (aspirin, statins, etc). Actigram creates a patient profile comprising context (location, time, schedule), physiological condition (clinical wearables), and psychological state (brief assays). The patient profile feeds a predictive engine that pushes timely messages to the patient's smartphone/smartwatch. The advice builds patient expertise, encourages mindfulness and motivation, and prompts specific actions. Patient performance, motivation, and adherence is monitored and used to both prompt social support from loved ones and physicians and to improve interventions over time.
